Use, as appropriate, the codes as appearing in Part 3.C of this Annex I identifying the diseases, that have been tested in accordance with the protocols of this Part 3.C or using tests for diseases requested by the Member State of destination.(10) In this case the health certificate has to be accompanied by the official document on quarantine and test conditions in Annex I, Part 1 to Decision 79/542/EEC (model “CAM”).(11) Officially tuberculosis/brucellosis free regions or herds recognised as equivalent to the requirements laid down in Annex A to Council Directive 64/432/EEC and which appear in column 6 of Part 1 of Annex I to Council Decision 79/542/EEC (as last amended), with the entry “VII”, as regards tuberculosis, “VIII”, as regards brucellosis.(12) Tests carried out in accordance with the protocols that, for the disease concerned, are described in Part 3.C of this Annex I.
